Who is the collective affiliation meant for?
Do you have at least 5 employees (EEA nationals, unless you have a registered office in Belgium) working outside the EEA(New window), Switzerland or the United Kingdom? Then you can sign up for a collective insurance policy for them.

Please note! In case you want to affiliate a non EEA or Swiss national, the complete level of affiliation (17) can imply certain restrictions.

When is your staff protected by the Overseas Social Security?
Your employees are insured, provided contributions have been paid:
- while they work in a country outside the EEA, Switzerland or the United Kingdom, and
- while they are temporarily (no more than 6 months) seconded from their overseas post with an A1 certificate of Coverage (page in French)(New window) to an EEA country (with the exception of Belgium), Switzerland or the United Kingdom,
- during their contractual leave immediately following the periods mentioned above, and
- upon request of the employee, during a period of no more than 12 months immediately following one of these periods, to the extent that they did not exercise any other gainful activity. Please note that receiving unemployment benefit is considered a gainful activity.
You can insure yourself retroactively by paying a so-called 'instant premium'. This way you cover periods in the past for which your employees where not affiliated to the Overseas Social Security, but still met the conditions. Your employees can also choose to pay the contributions themselves.

Are you using the non-secure version of the form? In this case, please keep a scan of your proof of ID ready.
Please note that you will be asked to submit a series of documents while completing this form:
- if you are intervening as a mandatory holder for the policy holder, you must provide us with a signed mandate (in French)(.pdf-New window),
- if you have subscribed to complementary medical care insurance to which dependants have to be added, you might be required to add documents or additional certificates. To find out more about this complementary insurance, consult the Medical Care Insurance page on this website.
If you do not have these documents available at the moment, you can already complete the form., However, your request will only be processed once all the required documents have been provided.

Medical Care insurance: addition of dependants
Did you also add the Medical care insurance to your collective affiliation? If so, you or a member of your staff benefiting from your contract can include dependants by completing the online form.
